Checklist (Whichever is applicable)

• Application Form
• Affixed one passport size photo on the form. (Please attach an additional 12 copies)
• Certified copy of identity card / passport on A4 size paper (International students - please attach 4 sets)
• One copy of completed Health Examination Medical Report (Valid for 3 months before your registration) attached with all the original laboratory results, chest X-ray film and report.
• Certified copy of academic transcripts at SPM / GCE O-Level or equivalent
• Certified copy of academic transcripts at STPM / GCE A-Level or equivalent.
• Certified copy of academic documents at diploma / tertiary level - certificate, transcript and its corresponding grading system at the awarding institution - if applicable
• Certified copy of English Proficiency Test results (MUET, IELTS, TOEFL or others)
• All application forms must be accompanied with a processing fee of
  (i) RM30.00(Malaysian Students) or USD10.00(International Students) for pre-nursing and undergraduate
  (ii) RM50.00(Malaysian Students) or USD30.00(International Students) for post graduate

• Results will be announced two weeks after the closing date, although early offers may also be given. Admission into IUCN is competitive and, therefore, no applicant is guaranteed a seat
• The IUCN does not appoint any agents to recruit students on its behalf
